The short answer
Yes, in almost every case. A loft conversion changes the roof from a structure carrying only itself and the weather to a floor carrying people, furniture and partitions. The existing ceiling joists were never designed for that.
New floor structure
The existing ceiling joists are typically sized to hold up a ceiling, not to be a floor. New floor joists are usually installed, often spanning in a different direction and frequently supported on new steel beams at the ends. That steelwork then has to land somewhere that can carry it.
What happens to the roof
Removing or altering rafters, purlins and struts changes how the roof holds itself up. Traditional cut roofs rely on the parts bracing each other, and taking one element out transfers load somewhere that may not be ready for it. Trussed roofs cannot be altered at all without designed replacement support.
Dormers, rooflights and hip-to-gable
Each changes the structure differently. A dormer needs trimming around the opening and support at the head. A hip-to-gable rebuilds an entire roof plane and changes how the roof is braced. Rooflights are the least involved but still need the rafters trimming properly.
The bit people forget
The load has to reach the ground. New steel beams concentrate load at their bearings, and those points continue down through the building to the foundations. A loft conversion is a whole-building question, not a top-floor one.
Where to start
Send the existing drawings if you have them, or photographs of the loft space showing the roof structure and the direction the joists run. That is usually enough to say what the job involves.
